Volume 12, Number 25 — August  2, 2023

Dear Healthcare at Home Colleague,

With the announcement this week that Epic will incorporate Microsoft's AI-powered clinical application, Nuance Communications, into its physician notes system, the question we have been following becomes a little closer to home. "Is AI a net positive or negative for healthcare?" We assigned Heather Thompson, our resident AI expert, to run some tests. She presented a rare condition with which she is familiar to three different conversational AI programs and had them diagnose her hypothetical patient, recommend a care plan, and then critique each other's solutions.

Two revelations came to light, both of which we consider critical. First and foremost, not all AI tools are alike, nor will they produce the same results. Secondly, instructions given by the human, so-called AI prompting, must be 100 percent complete, specific, and accurate. It is no wonder the new job title, "AI Prompt Engineer," is commanding salaries in the mid-six figures. (Keep that in mind if you have a high school senior looking for career ideas.)

Resist the temptation to take this movement lightly. Artificial Intelligence is already in healthcare and rapidly moving to in-home care, with consequences both exciting and grave. We devote two articles to it this week, one describing Heather's experiment, the other my reflections on what it means.

Florida Judge Changes Home Care

Guest author and home care attorney Elizabeth Hogue watches important court cases for us. The one she reports on this week is a big one. A judge has ordered Florida Medicaid to provide in-home care to disabled children to keep them out of institutions. Will Florida agencies be able to staff the additional shifts? Will such court orders start appearing in other states? Wait and see.
