Dear Healthcare at Home Colleague,

Medicare HHAs are hoping their EMR software will be ready when the new PDGM payment system starts in January. We sent senior staff writer Darcey Trescone to talk to a few vendor product managers to find out. The general consensus is that they regard it as a luxury to have a final rule in place so long before its deadline date. They know they have a lot of work to do but are confident they will be ready in time. Read Darcey's report "Software Vendors Discuss PDGM Perspectives and Preparations."

We are heavy on Medicare news this week but some of it will be of interest to private duty providers as well. CMS administrator Seema Verma is launching a program she hopes will make hospitals safer places. Be sure to read her open letter by clicking on "CMS Administrator Announces..." below. Her plan will have an impact on the entire post acute care community.

Patient data interoperability is a goal that concerns home health, home care, and hospice providers alike. Every health system executive has told us they will not consider a post acute care partner if they do not have an EMR system that can elegantly exchange patient data with their own EMR. If partnerships are in your agency's plans, be sure to see the proposed rule from the Department of Health and Human Services. The public comment period has just opened. In our reprint of the HHS announcement, we have a link to the entire proposed rule and to the public comment page.

Private Duty home care providers will be interested to read the announcement from one of our sponsors. Home health agencies and hospices are plagued by audits! The United Program Integrity Contractors (UPICs) are hard at work. RAC auditors are now back on the job, too. Plus, there are ADRs, scrutiny from the OIG, and more! What should providers do to avoid being in the crosshairs or to get out of the line of fire?

The answer, of course, is documentation. For home health agencies, auditors regard whether patients are homebound and whether care provided was reasonable and necessary as “low-hanging fruit.” Hospices are likely to face denials related to whether patients are terminally ill. 

Home health agencies should continuously document regarding homebound status. Hospices must focus documentation on whether patients continued to be eligible for the hospice benefit. Both types of providers should “tell the story” of patients’ conditions so that care is clearly reasonable and necessary. 

If providers are targeted by auditors, they should carefully review and make additions and changes consistent with applicable standards before submitting records.
