Dear Healthcare at Home Colleague,

Both Home Care and Home Health providers will find something educational in this week's news, Hospice too. We met a lot of people and gathered a lot of information at last week's NAHC Conference & Expo. I'll summarize the news for you here but you can also scroll down and click on the headlines to go directly to the stories most pertinent to you. First let me mention how terrific it was to meet so many of you in Seattle. Your gratitude for our efforts was gratifying and we will gratefully keep grinding away at it.

One more reminder, to vendors and consultants with products and services for home care, home health, and hospice. We are putting the final touches on our Home Care Buyer's Guide. Nearly 1,000 people used the original version and the next iteration will be even more useful. Every vendor we know of will at least have their name listed. We interviewed the newest partner in the Honor Care Network. The San Francisco company with the unique partnership plan for private duty home care providers has expanded into Michigan and Ohio. The Cleveland-area founder and CEO we interviewed was quite enthusiastic about the relationship.

For Medicare providers, we continue our series of interviews with reimbursement experts about PDGM. This week, staff writer Darcey Trescone interviews SHP's Chris Attaya.

Both Medicare and non-medical providers will want to at least glance at our NAHC Exhibit Floor summary. We toured the entire room and found new products, new vendors, and a couple of big new announcements from familiar vendors. Oh, and the food NAHC had the Seattle Convention Center provide was unusually tasty and healthy this year as well. Thanks, Bill.

A couple of those news items released at the NAHC meeting were worthy of their own articles. Be sure to read about new partnerships between Swift Medical, a wound care technology company, and two EMR technology companies whose names you will recognize. (Hint: their display ads can be found on this page.) The other item is about Wellsky, which made our "most-read" list last issue with the announcement that they have acquired private duty software company ClearCare. They announced a new partnership with Forcura.

Worth the trip

While I am enjoying the chance to stay home for a time, and enjoy the October Colorado snowfall, I am looking forward to meeting subscribers when I moderate a panel on PDGM and Health Information Exchanges at the Annual Conference of the New Mexico Association for Home Care & Hospice. Albuquerque, November 7. On December 12, I will keynote a technology day seminar for the Ohio Conference of Home Health and Hospice in Columbus. More on that later.

Home health agencies and hospices are plagued by audits! The United Program Integrity Contractors (UPICs) are hard at work. RAC auditors are now back on the job, too. Plus, there are ADRs, scrutiny from the OIG, and more! What should providers do to avoid being in the crosshairs or to get out of the line of fire?

The answer, of course, is documentation. For home health agencies, auditors regard whether patients are homebound and whether care provided was reasonable and necessary as “low-hanging fruit.” Hospices are likely to face denials related to whether patients are terminally ill. 

Home health agencies should continuously document regarding homebound status. Hospices must focus documentation on whether patients continued to be eligible for the hospice benefit. Both types of providers should “tell the story” of patients’ conditions so that care is clearly reasonable and necessary. 

If providers are targeted by auditors, they should carefully review and make additions and changes consistent with applicable standards before submitting records.
