Dear Healthcare at Home Colleague,

One of things public and private payers are trying to get post-acute care providers to do is coordinate patient care with other healthcare providers. Government officials and independent healthcare consultants have determined that assigning case managers to coordinate care as patients move from venue to venue has the twin benefit of improving patient outcomes, avoiding medical errors, and controlling costs.

We at The Rowan Report have heard of efforts around the country to create care coordination programs, going back as far as our 2011 coverage of the Community Health Initiative Network in Las Vegas. Rarely, however, have we seen a program as efficient as the one we feature this week.

Whether you provide services to Medicare or Medicaid beneficiaries, dual-eligibles, or patients who pay you directly or through their insurance policies, please take a look at "Hospital-to-Home Program Saves Lives, Slashes Costs." This care coordination program can serve as a national model; the home care agency CEO who helped design the program is open to speaking with colleagues about it.

Amid rumors and denials of rumors we are hearing here at Lincoln Health's "Senior Care 360" conference outside Washington DC that PDGM will be delayed, we continue to bring vendor insights and resources to help Medicare providers prepare for the likely 1/1/20 deadline. This week, staff writer Darcey Trescone interview principles from Wellsky and Axxess to learn about their predictions and preparations for their respective clients.

As every post-acute provider continues to struggle with staff recruiting and retention, we found a useful report from researchers at PHI and Caring Across Generations. Together, the organizations have released a report outlining nine areas where LTSS social insurance programs at the state level can improve the quality of direct care jobs.

In this week's Vendor Watch column, we have news about a multi-million dollar private equity investment in Sandata and two new executives appointed to manage Toronto-based AlayaCare's continued move into the U.S. private duty home care market.
