Their customers may never again inadvertently send a caregiver or nurse to an empty house. Underserved populations in Minnesota will be getting better care thanks to the partnership between the state's largest not-for-profit, in-home care provider and Minnesota Community Care, a St. Paul-based clinic that provides full-service health care for all people regardless of income or insurance status. At the core of the partnership is homecare-assisted telehealth appointments and in-home care management provided to the federally qualified health center by Accra.
