Dear Healthcare at Home Colleague,

This week's issue arrives in your inbox on a historic day. Changing the resident of the White House only happens every four or eight years, lacking a death or resignation. With each transition, it is not just the White House but the Cabinet Secretary offices that welcome new residents. That is where the changing of the guard will affect us most.

How will California Attorney General Xavier Becerra run the Department of Health and Human Services? What impact will his Assistant Secretary, Pennsylvania Health Secretary Rachel Levine, have? If confirmed, she will be the first openly transgender, Senate-confirmed federal official. Who will be chosen as CMS Administrator? Who will run the Office of the Inspector General? Will the ACA be expanded? Will a public health insurance option be added to it? Will Medicaid expansion continue or contract?

Today reminds us there is a thin line between excitement and terror. The way things are going, we may know the answers to all these questions soon. Still, you know the old Chinese curse, "May you live in interesting times."

The lobbying organization that represents health insurance companies, AHIP, sent us their scathing criticism of a CMS final rule intended to streamline the prior authorization. They are upset about not only the rule itself but the way CMS shortened the public comment period from the customary 60 days to 14. Put on your oven mitts and sunglasses and click on "Payers Livid About CMS Final Rule." This article also includes the full text of the CMS announcement.

Also from CMS, plans for insurance companies that participate Medicare Advantage have been announced for calendar year 2022. Our summary of the plan will be important to providers with MA contracts.
