Volume 6, Number 45 September  6, 2017

Dear Healthcare at Home Colleague,

I have once again been experiencing in-home care from the other side. Kudos and gratitude to Barry Berger and Accredited Home Care for the exceptional care they are giving my mother in Southern California. Mom says Monique is the best nurse ever and she loves Esther, "the bath lady."

Be sure to scroll down to the bottom of this page to learn about the newest entry in our Free Webinar announcements. Healthcare at Home pioneer Marilyn Harris is a founder of the International Home Care Nurses Organization and the editor of "Home Health Administrator's Handbook." You can read our review of the Sixth Edition of her masterful work here.

In This Week's News:
As the U.S. healthcare system walks a tightrope between the Value-Based Purchasing goals of the Affordable Care Act and the uncertainty created by dysfunction in Congress, many look to their software providers for guidance. All of the major vendors are focused on interoperability and we are speaking with each of them, one by one, to learn their plans. This week, Netsmart's EVP Kevin Scalia informs us about the behavioral health software company's strategy for its rash of home health and long term care software vendor acquisitions: Allscripts, HealthMEDX, and DeVero.

Hurricanes are on everyone's mind this month and our thoughts and prayers are with all the Healthcare at Home providers and their patients in Texas and Florida. We have two related news items. ElevatingHOME has initiated an effort to direct donations to two Texas home health and hospice associations. We have the essentials of the effort and a link for those who would wish to make targeted donations. Also, our telehealth reporter Audrey Kinsella speaks with organizations using telemedicine systems in Houston and Dallas shelters to connect children and the elderly with physicians located outside the danger zone.

Please do not forget our request to readers to keep us informed about developments in each state to comply with the EVV mandate. If you receive a notification from your state or if you have the contact information of the state official charged with compliance, please forward it to editor@homecaretechreport.com. You will help us help the entire Healthcare at Home community.

Lastly, meet Playmaker's new VP of Sales, Holly Miller, and experienced healthcare sales veteran from Middle Tennessee State University.

Have a great week. Keep making a difference!

New NAHC President Launches Top to Bottom Reorganization
The National Association for Home Care and Hospice announced last week that its board of directors has appointed Bill Dombi as Interim President. Immediately curious about how long an "interim" is and what might be in store for the association after the untimely passing of its 35-year and only president, we called Bill and were graciously given some time during what must have been an extremely busy week for him. He explained the reorganization project dubbed "NAHC 2.0."

Giving and Getting Referrals from Hospitals

Featuring: Elizabeth E. Hogue, Esq.

Date: Wednesday, May 8
Time: 1:00 - 2:30 pm EDT

Encore date: May 15
Option to purchase CD & handouts

Information and Registration

Referrals from hospitals may be even more important for agencies under the Medicare Program's Patient-Driven Groupings Model (PDGM). Hospitals also remain a major source of referrals for hospices and HME, amid increasing competition. Private duty agencies have a key role to play in the discharge of patients to home and referrals from hospitals are expanding rapidly. All types of providers need to know how to get more referrals from hospitals without breaking the law. And hospital discharge planners/case managers need to know the rules so the referral process remains in compliance!

This teleconference will address crucial questions for both providers and discharge planners, such as:

  • Can home care providers participate in discharge planning meetings?
  • Should home care providers have access to case managers and patients prior to discharge?
  • What do regulations and interpretive guidelines for discharge planning say about giving and getting referrals?
  • How can providers and case managers use Preferred Provider Agreements?

Extensive handouts will be provided, and there will be time for comments, questions and answers.

Date: June 6, 2019
Time:
12:00pm MT
Fee: Free

Learning Objectives:

1. Define the problem: be clear on the problem to be solved with telehealth initiatives.

2. Sponsorship: identify the right sponsorship, be clear on what is specifically needed from each level of sponsors.

3. Reinforcement: start with positive reinforcement, move to negative reinforcement on the clearly defined behavior changes

Jan Ground PT, MBA, has been leading clinicians and other healthcare personnel through change for more than twenty years. She led the virtual care work for the Colorado Permanente Medical Group and was the Innovation Hunter for the Colorado Region of Kaiser Permanente (KPCO). Most of her work supported innovation initiatives to enable people to get care anywhere and providers to provide care from anywhere. Areas of focus have included video visits, remote patient monitoring, e-visits, as well as the use of mobile apps and social media in care delivery. In all of these efforts, motivating providers to provide care differently was the most significant barrier to success. Jan has trained physicians and staff inside and outside of KP in Innovation, Meeting Management, and Leading People through Change. Jan has degrees in psychology, physical therapy, exercise science, and business administration from Middlebury College, Middlebury, VT; Emory University, Atlanta, GA; and the Univ of S. FL

The home care and hospice industry is increasingly competitive. The shift from “volume” to “value” means that agencies must increasingly focus on quality outcomes and prove their “worth” to customers and referral sources alike.  Agencies must have a robust quality program in place that identifies, tracks, and trends key metrics against agency goals.  Successful agencies not only have visibility into their current performance, but they continually modify and develop new strategies for improvement, while engaging their clinicians.
