Dear Healthcare at Home Colleague,

I always pick up valuable ideas when I attend state association conferences. This week's meeting of the Ohio Conference for Home Care and Hospice was no exception. Here is one example. Arnie Cisneros, PT, told Ohio Medicare providers that one in three of them will be forced out of business by the year 2025. The president and founder of Home Health Strategic Management quoted some of his executive-level clients in large health systems, who are preparing for value-based payments and payment bundling, as saying that 85% of care will be delivered in the home prior to or following hospital stays by that year. These executives will only allow their health systems to partner with the top-rated in-home care providers. "Reducing the total number of home health agencies is all part of CMS's plan," he told them.

In this week's news: Mediware, the parent company of Fazzi Associates, Rock-Pond Solutions, and Kinnser Software and about 30 other software products, announced during its user group meeting this week a major re-branding effort that includes a corporate name change.

Partners Health has begun to promote this year's Connected Health Conference, set for October 17-19 at the Seaport World Trade Center in Boston. See inside for conference details and registration information. The main conference message is to promote a personal approach to health.

Speaking of Fazzi Associates, a new series of online courses is about to begin. Students can earn accredited certification in Home Health Case Management and Hospice Case Management. Members of ElevatingHome can register for a discount.
