Dear Healthcare at Home Colleague,

I spent some time reading the final rule regarding the changes coming in 2016 to Medicare hospice payments. Well, OK, I read the CMS summary. The 221-page original document was a little daunting. But I did call on an experienced hospice nurse and administrator to help me understand it. I hope my interview with her is helpful to you.

This nurse happens to be currently working with Medalogix, a tech company that we have reported on before but never in great detail. Intrigued by her description of how early adopters of this company's data analytics engine have benefited, I sought out additional interviews with one of those customers and then with her CEO. I think you will find these interviews informative as well. This is not just another home care software application, though it can be integrated with your EMR. It is unique in its construction and in its mission. The article is titled, "Data Analysis Tool Identifies Home Care Patients Ready for Hospice." Don't miss it.

In other news this week: The activities and practices of insurance companies are important to most of you so we try to keep an eye on them from time to time. What do you think the consequences will be of the announced and/or rumored mergers and acquisitions that seem to be imminent for Aetna, Anthem, Humana, UnitedHealthCare, and Cigna? Larger bureaucracies for you to contract with? Certainly. Layoffs in Louisville and Hartford and other places? Guaranteed. Lower premiums for your patients and employees and higher payments for your services? We'll see. But more on that next week.

This week, our focus is on Medicare Advantage (because we only wanted to scare you a little before we have to scare you a lot). Did you know that it has become a common practice to send physicians into homes of Medicare Advantage enrollees to assess their health level? Since this practice started, MA profits have skyrocketed while the quality and amount of care provided has remained the same. How, you ask? Check out the guest article by Fred Schulte of the Center for Public Integrity, titled "Medicare Advantage Plans Pad Charges on Home Visits, Whistleblower Says," in which he describes the suit by a former insurance employee and the disappointing story behind it.

Lastly, two more important stories. Guest author Roger McManus offers number six of his eight-part series on Reputation Management. This one helps you understand better how to -- and how not to -- interact with Yelp and other online review services. And we interview Dr. Andrey Ostrovsky, the creative genius behind Care at Hand, a product we have long been high on but which has been in test mode for a while, but which will now benefit from a substantial investor boost. Keep an eye on this one. It will have a major impact on hospital readmissions.

Immediate and urgent need for Interim Director of Billing (outside of Chicago). They can be based anywhere. We would fly them in on Monday and home on Fridays.
