Why "Thrive?" You have heard me predict for years that CMS is trying to reduce the total number of healthcare at home providers from over 12,000 to fewer than 8,000. That prediction is already coming true. At the same time, non-medical, private duty providers and hospices are on the rise. I will be using my 90 minutes to describe characteristics of the "Surviving 50%," agencies that will still be around after CMS gets done with its pruning project. I will try to remain upbeat and optimistic, and add my usual smattering of humor, but be warned, I am coming to Columbus to scare you...and you need to be scared.

Speaking of fear. We were critical last week of the OIG report that claims 52% of home health payments are improper. A reader pointed out to us that we should have added that it does not matter whether that number is inflated, as we said it is by your MAC's extreme interpretation of the ACA's Face-to-Face law. And that reader was right. What matters is that this is what the OIG (and CMS, and MedPAC, and Congress, and the President's advisors) believes. It is upon that belief that regulators and lawmakers will base their next actions.

National association lobbyists may be able to change regulator and lawmaker minds someday, though they have not so far, but you have to survive today. You have to be more than compliant. You have to be perfect, beyond reproach, above doubt in the face of those who are paid to doubt you, to find the slightest flaw in your assessment, your care plan, your visit note, your very personal integrity. Perfection is not easy, but it is possible. Let's talk about it in Columbus.

In this week's news: The one thing that will change payer attitudes is research that proves once and for all that in-home healthcare saves payers more than it costs them. Don't miss our lead story this week about a trio with unquestionable credibility finally initiating that research: ClearCare, Right at Home, and Harvard Medical School.

We also have news from two vendors, Allscripts Homecare, now known as Netsmart Homecare™, and Relias Learning. Both are hiring. One introduces a new home care division manager, the other outlined plans to create 450 new positions.

Lastly, guest writer Michelle Boasten did some research for us into the story we reported couple weeks ago about startup HomeHero's partnership with Cedars-Sinai Hospital in Los Angeles. It didn't turn out they way we thought it would.
