Volume 8, Number 21 May  1, 2019

Dear Healthcare at Home Colleague,

We here at the Rowan Report are often regarded as a source of information for Medicare-certified Home Health and Hospice providers. This week, however, we bring you a news analysis that is possibly the most important report we have ever written for the non-medical, personal care side of our healthcare sector. If you are in that sector, scan through the headlines below this letter and click on our interview with Bob Roth, managing director of Cypress Home Care in Phoenix.

Roth has decided to have Cypress join the Honor Care Network. In the past, we have relayed to you both positive and negative reader opinions about Honor's reincarnation as a strategic partner instead of as a full-service home care agency of its own. With the jury still out, in our opinion, we were anxious to learn Bob's decision process.

Frankly, we were impressed. You will read that he sees Honor's approach as, among other things, a solution to a looming crisis...not a problem, a crisis. Under current wage limitations and labor laws, in less than 10 years there will simply not be enough caregivers to serve a population that is aging and that values remaining at home as long as possible. Our conversation with Bob Roth was wide-ranging but we think we have captured the essence of his journey toward this decision. Plus, the article includes the frightening demographic statistics that drove him to look for a solution in the first place.

In other news:

Senior staff writer Darcey Trescone interviewed Jeremy Powell, CEO and Founder of Acclivity Health Solutions, a platform for connected care communities that built a reputation in acute care and is now turning its attention to post-acute, specifically Hospice. Big data, Darcey discovered, will be crucial to the effort to improve care outcomes for patients with advanced illness.

This issue's Vendor Watch column has several announcements. A new Hospice Academy for Texas Association for Home Care & Hospice members; a mileage calculator add-on for Wellsky users; and a new COO for a reorganized HHAExchange.

Guest author Roger McManus, MBA, teaches a marketing trick you should try. There is a way to appear as the only result in a search when someone is looking for home care services in your market area on Google. His primer shows you how to do it, step by step.

For the IT Department, we share what we learned about the next Windows 10 update, due this month. We don't usually do this but this one is big. So big, Microsoft will not automatically download it to your PC. You get the option to upgrade or not, but only for 35 days. Note to IT support staff: long nights ahead. While waiting for the as yet unannounced release date, start checking available hard drive space on every PC. Not only does 10 absorb about 30GB, now it will set aside an additional 7GB of reserved space to allow for the smooth installation of future automatic updates.

Have a great week. Keep making a difference! I hope to meet some of you at next week's annual meeting of the Home Care Association of New York State in Saratoga Springs.

Giving and Getting Referrals from Hospitals

Featuring: Elizabeth E. Hogue, Esq.

Date: Wednesday, May 8
Time: 1:00 - 2:30 pm EDT

Encore date: May 15
Option to purchase CD & handouts

Information and Registration

Referrals from hospitals may be even more important for agencies under the Medicare Program's Patient-Driven Groupings Model (PDGM). Hospitals also remain a major source of referrals for hospices and HME, amid increasing competition. Private duty agencies have a key role to play in the discharge of patients to home and referrals from hospitals are expanding rapidly. All types of providers need to know how to get more referrals from hospitals without breaking the law. And hospital discharge planners/case managers need to know the rules so the referral process remains in compliance!

This teleconference will address crucial questions for both providers and discharge planners, such as:

  • Can home care providers participate in discharge planning meetings?
  • Should home care providers have access to case managers and patients prior to discharge?
  • What do regulations and interpretive guidelines for discharge planning say about giving and getting referrals?
  • How can providers and case managers use Preferred Provider Agreements?

Extensive handouts will be provided, and there will be time for comments, questions and answers.

A new release of the Hospice PEPPER is available as of April 5. Participants will gain working knowledge of how to use their Hospice PEPPER.

Kimberly Hrehor, MHA, RHIA, CHC, PMP is a program director with the RELI Group. Date: June 6, 2019
Time:
12:00pm MT
Fee: Free

REGISTER NOW

Learning Objectives:

1. Define the problem: be clear on the problem to be solved with telehealth initiatives.

2. Sponsorship: identify the right sponsorship, be clear on what is specifically needed from each level of sponsors.

3. Reinforcement: start with positive reinforcement, move to negative reinforcement on the clearly defined behavior changes

Jan Ground PT, MBA, has been leading clinicians and other healthcare personnel through change for more than twenty years. She led the virtual care work for the Colorado Permanente Medical Group and was the Innovation Hunter for the Colorado Region of Kaiser Permanente (KPCO). Most of her work supported innovation initiatives to enable people to get care anywhere and providers to provide care from anywhere. Areas of focus have included video visits, remote patient monitoring, e-visits, as well as the use of mobile apps and social media in care delivery. In all of these efforts, motivating providers to provide care differently was the most significant barrier to success. Jan has trained physicians and staff inside and outside of KP in Innovation, Meeting Management, and Leading People through Change. Jan has degrees in psychology, physical therapy, exercise science, and business administration from Middlebury College, Middlebury, VT; Emory University, Atlanta, GA; and the Univ of S. FL
