Dear Healthcare at Home Colleague,

I have been promising to tell my personal story as a total knee replacement patient. Six weeks may not be long enough to tell the entire story but I am post-discharge from home health and back on the road delivering presentations and attending conferences. The next 6, 12 or 46 weeks would be far less interesting to readers of this report.

It was a fascinating experience. I learned more in two months about what many of you do than I had learned since entering this field in 1993. I have always admired nurses; now my admiration extends to therapists. My story is a little long but my intent was to conduct a study of what it is like on the patient's side of this equation. Some of my observations may seem minor but what is obvious to one may be news to another.

I can't say I enjoyed the experience but I did enjoy writing about it. And I've been told that, someday, I will be glad I finally got it done. That day has not arrived yet. So if you see me grimacing at this week's BRG Healthcare Leadership Conference in Washington DC, feel free to offer a word of encouragement...but not too hearty a pat on the back.

Next week, we'll get back to real news, starting with the sigh of relief certified providers and their associations breathed when the Home Health Grouping Model was surgically removed from 2018 PPS changes. We will also continue to delve into the controversial EVV mandate in the 21st Century CURES Act.

Total Knee Replacement, A Patient's-Eye View
Since I wrote my first article for this report in the summer of 1998, I have often written about the way you take care of total joint replacement patients. From wound care nurses and physical and occupational therapists in the field to the software vendors that help them track a patient's progress, it is safe to assume that everyone reading this page has been directly or indirectly impacted by the dramatic increase in the total number of these surgeries. Today, I find myself on the other side of the scalpel. I took detailed notes before, during, and after my own October 24 total knee replacement procedure. I trust I am not assuming too much that some readers might be interested in my experience.

In March 2018, CMS released the draft of the latest version of the OASIS data set, OASIS-D. Modifications are being made to OASIS in order to comply with the IMPACT Act, to reduce data collection burden, and to improve formatting throughout the document. CMS is seeking approval from the Office of Management and Budget for a January 1, 2019 implementation date. Topics:

  • Addition of six new standardized items
  • The removal of 28 OASIS questions that will no longer be assessed.
  • Modification to M1311 to support a new standardized pressure ulcer measure.
