Volume 5, Number 42 October  26, 2016

Dear Healthcare at Home Colleague,

What we have here is a failure to communicate!

It became clear at last week's NAHC Annual Meeting that a consensus is building that patients are being harmed and money is being wasted by care silos. Especially in the non-acute care world, both pre- and post-hospital, the message was as large as mouse ears in Orlando. Failure to communicate (at best) or out and out competition (at worst) among home health, home care, hospice, assisted living, rehab facilities, skilled nursing facilities and other community-based healthcare services must end

Our lead article this week underscores the message. Netsmart has acquired HealthMEDX, bringing Long Term Care EMR software under the same umbrella with its home health, hospice and behavioral health offerings. But that announcement arrived after we had left Orlando, where the message had already been delivered.

Look, for example, at our second story, summarizing NAHC 2016 highlights and lowlights. We discuss the meaning of Long Term Care EMR vendor PointClickCare exhibiting for the first time. Next to their elaborate booth, Optima Healthcare Solutions, the leading provider of software to physical therapy companies, unveiled its just-released home health EMR. HeathWyse was there, in its own exhibit space, after having been acquired just the week before by another large PT software vendor, Casamba LLC.

Outside of the vendor space, we continue to hear reports of Skilled Nursing Facility organizations acquiring home health and home care agencies, creating highly competitive pathways to keep patients under one corporate roof as they transition from one care locale to another. Underscoring that trend, we also noticed Brookdale Senior Living, Optima's largest customer, also had a booth to promote its home health business unit.

This is a trend that is likely to continue. Let's all keep an eye on it. If I have missed any signals, let me know.

In other news:

  • We plan to always include non-skilled private duty home care in our watch over this developing trend. Correspondent Michelle Boasten attended a unique home care marketing conference — actually she likens it to a revival — last week. She writes about the number of Medicare agency owners there investigating "the other side."
  • We close the book, hopefully for the last time, on the story of the conviction and sentencing of an Amedisys VP who pleaded guilty to embezzling from his employer.
  • After months of contract bidding battles, CMS finally chose a new crop of bill collectors to reboot the RAC program. Guest author attorney Elizabeth Hogue analyses the new rules they will be bending in their quest to find improper payments and win their 12% commissions. Don't skip this article, there is actually some good news in it.
  • CMS just released the 2017 Home Health Final Rule and Kinnser has a free webinar about it already. Two experts, Sharon Harder, an executive management consultant, and Jill Dyera clinical consultant will show how the two sides of your home health operation depend on each other. It should be relevant to both business and clinical leaders. Click on the ad above.

Before I close, a special invitation. I will be one of the VIP healthcare at home experts making a presentation during the Sixth Annual "Compliance Cruise," January 15-22, 2017. This will be my first time on this popular event but I hear the Caribbean is a fabulous place to learn. And what a learning experience it will be. Joining me will be attorneys Liz Pearson and Lucian Bernard, Medicare consultant Jim Plonsey, financial consultant Patrick Brown, and sales and marketing expert Cheryl Peltekis, a collective 120+ years of experience keeping Medicare and Medicaid agencies in good standing with CMS and its contractors. Click here to see the details (and the tempting photos).

Have a great week. Keep making a difference!

New RACs and New Rules
Brace yourselves. RACs are back. This time there are more of them but, attorney Elizabeth Hogue explains, they will still pit their subjective definition of homebound status and terminal illness against yours. There is some good news in the new rules, however. Read on...

Netsmart Broadens LTPAC Reach With HealthMEDX Acquisition
This is not just another vendor consolidation announcement. This is one more indicator that a consensus is forming that non-acute care is bigger than home health care. Is it time for enhanced collaboration? Spearheaded by a national association with a larger, more inclusive umbrella?

Clear Theme Emerges at NAHC Meeting: "Come Together"
In our annual summary of what caught our eye in the NAHC exhibit hall, we detected not only new software but also an important new trend. The presence of so many skilled nursing facility and senior living providers may indicate they are telling us it is time for pre- and post-acute providers of all types to start working together.

Joel, Billy & TD...Make Room for the Hurricane!
Ever been to a tent revival meeting? Special correspondent Michelle Boasten attended Steve Weiss' "Hurricane Bootcamp" for owners of non-medical private duty agencies and came back believing that she had just seen one.

Amedisys VP Jailed for Wire Fraud
After pocketing more than $7.6 million over eight years, Michael David Pitts pleaded guilty last March and was sentenced last week to four years in prison.

The home care and hospice industry is increasingly competitive. The shift from “volume” to “value” means that agencies must increasingly focus on quality outcomes and prove their “worth” to customers and referral sources alike.  Agencies must have a robust quality program in place that identifies, tracks, and trends key metrics against agency goals.  Successful agencies not only have visibility into their current performance, but they continually modify and develop new strategies for improvement, while engaging their clinicians.
